Heidi Duckworth
MSN, APRN, FNP-C
Heidi Duckworth, MSN, APRN, FNP-C, is a board-certified Family Nurse Practitioner with 16 years of advanced practice experience in adult primary care, geriatrics, pain medicine, and complex chronic disease management.
Heidi’s clinical background includes caring for medically complex older adults in assisted-living, memory-care, and home settings, as well as extensive experience evaluating and managing acute and chronic pain, musculoskeletal and neurologic conditions, polypharmacy, and multiple chronic conditions. She also brings expertise in diagnostic evaluation, medication management, preventive care, care coordination, and patient and family education.
Known for her evidence-based and patient-centered approach, Heidi takes the time to understand each patient’s needs and develop thoughtful, individualized treatment plans. She is committed to ethical, compassionate care that supports appropriate treatment, promotes quality of life, and helps patients feel informed and supported throughout their healthcare journey.
